Swap creation fails

Got everything working perfectly except for 1 last thing: swap creation fails during VM creation. Any ideas?

When I boot up the newly created VM I see a 1GB /dev/xvdb device with 1 partition of type 83 (Linux). There is no entry in /etc/fstab for swap. Here's the creation log:

Adding DNS entry cwtest.{cloudmin zone}. for IP address {allocated ip} .. .. done

Starting up new Citrix Xen instance .. .. done

Regenerating SSH host keys .. .. done

Setting password for existing Webmin install .. .. password changed successfully.

Opening firewall ports for Webmin .. .. already open
