I was just browsing your site to figure out how to properly tell a user how to forward email. I think there is possibly some sort of issue with the way this page tells them because it didn't always work properly according to them. The logs seem to show that it does send mail but they had a hard time figuring it out so I told them just to log into Usermin to forward email.

Also, the main point of this post was to say if you take a look at this link you will notice several HTML issues on the page and I just thought I should point that out to you since I am sure you don't want that to be like that. =)
