When creating the Virtual Server -- make sure that the "Create MySQL login?" feature is selected.
Also, you may want to tweak the Account Plans... you can do that in System Settings -> Account Plans. There's a variety of database related features to tweak in there, but in particular, make sure that there's not a limit on the number of databases they can create.

Account plans was what I was missing (I'd done the MySQL stuff in Server Templates, but hadn't set it on the plans.
