Spamassassin incorrectly configured

Spamassassin filtering is being applied to domains even when the domain option boxes for "Spam filtering enabled?" are unchecked.

Spamassassin processing is apparently *broken* as well. I have "Action for messages classified as spam" as "Append to mbox-format mail file", with a parameter of $HOME/spam.occmail. But nothing is ever delivered into that user's mailbox. I checked the /var/log/maillog and found a number of these messages apparently related to Spamassassin processing. The relevant portion is here:
