When creating a database, I keep getting the error, "failed: Access denied for user ''@'localhost' to database 'mysql'"

Howdy -- you may want to review your MySQL settings in Webmin -> Servers -> MySQL.
You may want to review all the settings there, but you can check to see if you can change your password in "Change Admin Password", and that the admin user shows up as "root".
