2 more on the same server that did not get deleted....

Ok, I found the cause of this - it is a bug in Virtualmin, triggered only when the rarely-used automatic sub-domain deletion option is enabled. Since this is rarely needed, you should turn it off at Module Config -> Advanced options -> Delete all Apache virtual hosts in domain when deleting?

Is this reproducable reliably? If so, can I login to a system that is having this problem and see what is going wrong?

Sure but i dont know if it happens if the parent is deleted or just the child.
