So the issue is, if you create a mysql user like this: Edit Mail and FTP Users -> Add a user to this server. -> Allow access to databases. this is work fine.
But when we change the user password in the virtualmin, then that don't change the password in mysql, but change the linux user pass. And this cause some conflict in the customers brain :)

Thanks, that is a bug, which I will fix in the next Virtualmin release. The work-around is to add or remove access to a database at the same time the user's password is changed, which will cause it to be properly updated.
